A Case of Adenosquamous Carcinoma of the Ascending Colon
Summary
We report a case of adenosquamous carcinoma of the colon. A 70-year-old woman underwent a colonoscopic examination because of a positive fecal occult blood test. Colonoscopy demonstrated a type 2 tumor of the ascending colon, and a biopsy specimen showed poorly-moderately differentiated tubular adenocarcinoma. We performed a right hemicolectomy with D2 lymphadenectomy. The histopathology of the tumor demonstrated adenosquamous adenocarcinoma. Primary adenosquamous carcinoma of the colon is relatively rare and has a poor prognosis. Therefore, adenosquamous carcinoma of the colon may require strict follow-up. 要旨
患者は70歳,女性。主訴は便潜血検査陽性。下部消化管内視鏡検査を受けて,上行結腸に2型腫瘍を認めた。生検による病理診断は,低～中分化型腺癌と診断。以上の結果から,結腸右半切除術+D2郭清を行った。摘出標本の病理組織検査では,低分化腺癌と扁平上皮癌成分の混在が認められ,腺扁平上皮癌と診断された[pSS,ly0,v0,pN0,ow(-),aw(-),M0]。大腸癌は,そのほとんどが腺癌であり,腺扁平上皮癌は比較的まれな疾患である。この腫瘍の特徴は,病状が進行してから発見されることが多く,リンパ節への転移率が高い。腫瘍の発育速度が腺癌と比べて速く,予後不良と考えられている。自験例は術後6年を経過し無再発で生存中であるが,本疾患の特徴から慎重な経過観察が必要と考えられる。
